Overview

Bristol Myers Squibb (BMY) discovers, develops and sells medicines for serious diseases, with disclosed activities spanning oncology, hematology, immunology, cardiovascular disease and neuroscience. The company reports its commercial portfolio in two groups: a Growth Portfolio composed primarily of medicines earlier in their lifecycles, and a Legacy Portfolio containing Eliquis and products facing mature-brand or generic pressure. It earns revenue from net product sales plus alliance, royalty and other revenue. Q2 FY2026 revenue was geographically concentrated in the United States, which generated $8.99 billion, or roughly 69% of the $12.97 billion total; international markets contributed $3.98 billion. Specific customer concentration was not disclosed. The industry structure combines patent-protected products, regulated approvals, payer pricing pressure and eventual generic competition, making pipeline productivity and successful launches essential to replacing revenue lost at patent expiry.

What changed this quarter

The central change was a stronger revenue trajectory. Q2 FY2026 revenue increased 5.7% year over year to $12.97 billion, or 5.0% excluding foreign exchange. The Growth Portfolio produced $7.56 billion, up 15.0%, while the Legacy Portfolio declined 4.0% to $5.42 billion. This was not simply a currency benefit: Growth Portfolio revenue increased 14.0% excluding foreign exchange, and total company growth was 5.0% on that basis.

Growth was broad across newer products. Reblozyl revenue increased 29.0% to $735 million, Breyanzi rose 41.0% to $484 million, Camzyos advanced 60.0% to $416 million and Opdualag grew 23.0% to $349 million. Cobenfy increased 81.0%, although its revenue base remained modest at $63 million. Opdivo Qvantig contributed $261 million, compared with $30 million one year earlier. The principal weak spot within the Growth Portfolio was Opdivo, where revenue declined 3.0% to $2.49 billion.

The Legacy Portfolio decline would have been considerably worse without Eliquis. Eliquis revenue increased 22.0% to $4.48 billion, offsetting sharp erosion in products exposed to generic competition. Revlimid fell 49.0% to $425 million, Pomalyst/Imnovid declined 71.0% to $204 million, Sprycel dropped 27.0% and Abraxane declined 47.0%. Eliquis therefore remains both an important cash generator and a source of concentration risk.

Profitability improved sharply on a GAAP operating basis but not uniformly. Operating income was $4.09 billion, producing the supplied operating margin of 31.5%, compared with 14.5% one year earlier. GAAP EPS increased from $0.64 to $1.62, while non-GAAP EPS rose from $1.46 to $2.04. The comparison benefited from the absence of the $1.51 billion acquired IPRD expense recorded in the prior-year quarter and from lower amortization of acquired intangible assets.

Gross economics moved in the opposite direction. GAAP gross margin declined 120 basis points to 71.3%, which management attributed primarily to product mix. Selling, general and administrative expense rose 7.0% to $1.83 billion as the company invested behind new launches. GAAP research and development expense increased 15.0% to $2.96 billion, partly due to a priority review voucher and higher IPRD impairment charges. Free cash flow declined 13.2% to $3.09 billion from $3.56 billion.

The most consequential update was guidance. Management raised 2026 revenue guidance from $46.0-$47.5 billion to $49.0-$50.0 billion. The midpoint increased from $46.75 billion to $49.50 billion, or 5.9%. Non-GAAP EPS guidance rose from $6.05-$6.35 to $6.75-$7.00, lifting the midpoint 10.9% to $6.875. Eliquis growth guidance was also increased from 10.0%-15.0% to 20.0%-25.0%. Gross-margin guidance remained 69.0%-70.0%, while expected operating expenses increased from $16.3 billion to $16.5 billion.

Why it matters for the thesis

The quarter strengthened the case that Bristol Myers Squibb can manage the transition from mature medicines to its newer portfolio. The Growth Portfolio generated $2.14 billion more revenue than the Legacy Portfolio during the quarter and grew fast enough to overcome the latter's generic erosion. That is the operating evidence required for a portfolio-renewal thesis: newer medicines are no longer a small offset to older brands but the larger disclosed portfolio.

The quality of the growth remains mixed. Camzyos, Reblozyl, Breyanzi and Opdualag delivered substantial increases, but Eliquis supplied $801 million of year-over-year revenue growth by itself. Eliquis represented nearly one-third of total quarterly revenue, and management's higher full-year outlook partly rests on raising the product's expected growth range. Portfolio renewal is progressing, but the company has not yet eliminated blockbuster concentration.

The 120-basis-point gross-margin decline also matters. Management said product mix caused the contraction, and unchanged full-year guidance of 69.0%-70.0% indicates it does not expect an immediate reversal strong enough to lift the annual range. Higher launch investment is economically sensible if it supports durable growth, but SG&A rose faster than total revenue and expected full-year operating expenses increased by $200 million.

Cash generation was the other soft point. Quarterly free cash flow fell to $3.09 billion even as accounting earnings improved. One quarter does not overturn the supplied trailing free cash flow of $11.4 billion or its 8.8% yield, but durable deleveraging requires cash conversion as well as stronger EPS. The quarterly table shows net debt of $34.14 billion, while the company's broader calculation including marketable debt securities reported a $31.66 billion net debt position at June 30. Either measure leaves meaningful financial leverage.

On balance, the guidance increase outweighs the margin and cash-flow softness. Revenue momentum was broad enough for management to raise its full-year range by $2.75 billion at the midpoint, while the EPS midpoint rose even faster. The quarter therefore improves confidence in near-term execution, but it does not resolve the longer-term questions around patent exposure, pricing pressure and pipeline commercialization.

What BRISTOL MYERS SQUIBB CO is worth after the print

There is no prior SageNoodle fair value to carry forward, so this update establishes an initial valuation. We use the midpoint of management's updated 2026 non-GAAP EPS guidance as the base earnings measure because the quarter directly changed that outlook. The method is an earnings multiple rather than an enterprise-value framework because EBITDA was not disclosed in the supplied materials; accordingly, the snapshot EV/EBITDA field is shown as unavailable with a value of zero.

The base case applies an assumed 11.0 times multiple to $6.875 of 2026 non-GAAP EPS, producing fair value of $75.60 per share. The multiple is an explicit assumption rather than an imported peer comparison. It reflects a business with strong current cash generation and an expanding Growth Portfolio, balanced against substantial debt, generic erosion, product concentration and recurring acquisition-related adjustments.

At the current price of $63.75, the base case implies 18.6% upside and meets the stated threshold for an Undervalued verdict. The price also equals 9.3 times the updated guidance midpoint. The scenario range is deliberately wide because the next valuation step depends less on one quarter's accounting earnings than on whether newer medicines can replace legacy revenue without persistent margin dilution.

The bear case assumes $6.50 of normalized EPS, below current non-GAAP guidance, and an 8.5 times multiple. That produces $55.30 per share and represents weaker Eliquis growth, continued generic erosion and limited operating leverage. The bull case assumes $7.25 of normalized EPS and a 13.5 times multiple, producing $97.90 per share. That outcome requires sustained double-digit Growth Portfolio expansion, successful launches and evidence that gross-margin pressure is temporary.

What could prove this wrong

The clearest disconfirming evidence would be a slowdown in the Growth Portfolio before it becomes large enough to absorb legacy losses. Opdivo revenue already declined 3.0% this quarter, and several of the fastest-growing products remain much smaller than Eliquis or Opdivo. Failure to convert regulatory milestones into commercial revenue would weaken both the earnings assumptions and the valuation multiple.

A second failure point is Eliquis. The raised 20.0%-25.0% growth outlook materially supports management's full-year revenue guidance, but it also increases dependence on one product. Pricing controls, reimbursement changes, competition or an earlier-than-expected decline would expose the underlying contraction in the rest of the Legacy Portfolio.

Margins provide an early warning signal. Gross margin declined despite higher revenue, SG&A increased with launch spending and management raised the operating-expense outlook. If product mix keeps gross margin near or below the bottom of the 69.0%-70.0% annual range while launch expenses remain elevated, revenue growth may not translate into the normalized EPS used in the base case.

Finally, free cash flow and leverage must improve together. A repeat of the quarter's 13.2% free-cash-flow decline would reduce the capacity to repay debt, fund the pipeline and return capital. The thesis would be materially weaker if stronger adjusted EPS continued to coexist with soft cash conversion and net debt remained above $30 billion.
